- Authors: Last name first followed by initials (without periods); presenter name should be indicated by an asterisk.

- Affiliations: Name of institution, city, state/province, country (no postal box numbers, street addresses, or postal codes). Multiple affiliations may be indicated by corresponding numbers in parentheses in the author line and the affiliation line.
